Describe the problem in context
Include the page URL, browser or device, the expected result and what happens instead. List the steps needed to reproduce it. Note whether the issue affects one page, a form, an integration or the whole website.

Identify recent changes
Tell the team about recent content edits, software updates, DNS changes or third-party configuration changes. Avoid repeating an update or changing several settings at once while a live issue is being diagnosed.

Agree how an update will be checked
Before a change, agree the backup or recovery approach and the checks that matter to your website. These might include key pages, enquiry forms, customer access and checkout. A staging workflow can be used where it is available in your agreed setup.

Plan improvements against a useful outcome
For new work, describe the visitor or business outcome you want to improve. Separate urgent faults from content changes and new features so the team can discuss priority, scope and any additional quote.
